Who This Is For
Some people already hold their medical history. A folder of discharge summaries, a records request they made themselves years ago, printouts from a practice they have since left, a box of papers inherited from a parent. If that is you, there is nothing left to request and no reason to pay us to request it.
Everything reaches us as digital files. Scans and photographs of paper are fine, and a phone camera is good enough, but we cannot accept anything sent by post.
