Your GP record holds the letters your hospital sent back. It does not hold the hospital's own file.

Start Here: You May Already Have What You Need

This is the part most people are surprised by, and it is worth reading before you fill anything in. Your GP record is not just a record of your GP visits. Information flows back into it from across the health service, and for most people it is the single fullest account of their health that exists anywhere.

Ask for your full GP record and you will typically find referral letters, consultant clinic letters, hospital discharge summaries, test and imaging reports, prescriptions and immunisations, often going back decades and often including scanned handwritten paper.

So if what you actually want is "what did that consultant say" or "what happened when I was in hospital in 2011", there is a good chance the answer is already there, in a record you have a legal right to today. That is the service we run now.

What Only the Hospital Holds

Some things genuinely never reach your GP, and no amount of asking your practice will produce them. These are the records this page is about.

Full Inpatient and Operation Notes

Your GP receives a discharge summary after a hospital stay. The ward notes, the operation note, the anaesthetic chart and the nursing records stay with the trust.

Mental Health Trust Records

Mental health trusts run entirely separate systems. Clinic letters may reach your GP; therapy notes, care plans and risk assessments do not.

Maternity Records

Antenatal, birth and postnatal notes are held by the trust that delivered your care, in far more detail than anything your GP was sent.

Private Clinic and Hospital Files

Bupa, Nuffield Health, Spire and the rest hold their own records, and often keep each division on its own system. Nothing obliges them to copy your GP in at all.

Diagnostic Imaging

The written report usually reaches your GP. The images themselves live on the hospital imaging system, and they are the one thing we could not organise for you even if we held them.

Sexual Health and GUM Clinics

These carry enhanced confidentiality by design. Unless you specifically consented to disclosure, your GP record may hold no trace of an attendance at all.

Why This Is Not on Sale Yet

Getting your GP record is one request to one organisation, and we have done it enough times to know what it costs and how long it takes. Records held elsewhere are a different shape of problem, and we would rather explain that than quietly price around it.

  • · Every organisation is a separate Subject Access Request, with its own form, its own department and its own idea of what counts as proof of identity.
  • · The legal clock is one calendar month, extendable to three where a request is complex, and hospital trusts use considerably more of it than GP practices do.
  • · Older records may sit in an off-site archive, on a decommissioned system, or across several trusts if your care moved.
  • · Nobody can promise in advance what any given organisation will release, or how complete it will be when it arrives.

None of that makes it impossible. It does mean we want a clear view of what people are actually asking for before we put a price and a promise next to it, which is what this register is for.
