£332.50 net + £66.50 VAT. Paid only after your record is delivered. No upfront payment.

Requesting your starter pack costs nothing and commits you to nothing. It explains exactly how the service works so you can decide, with no obligation, whether to go ahead. By submitting, you agree to our Privacy Notice.

Recommended Option A: We collect your records for you

Chronicle Health acts as your authorised representative and submits the Subject Access Request to your GP practice.

· You authorise us to gather your data and share the relevand documentation
· We handle the requests, any questions or chasing, and the receipt of records
· You receive your organised health record at the end

Option B: You collect your records

You submit the SAR yourself using our template and guide, then send us the records to organise.

· You contact your GP practice using our template
· You prove your identity to the GP, and handle any admin or questions
· You upload the records you receive; we organise from there
