Every occupational medical starts the same way: a questionnaire asking for a medical history spanning your whole life, assessed by a doctor who has never met you. Dates of diagnoses. Past episodes. Medications started and stopped. Referrals, investigations, results.

Almost everyone answers it from memory. A consultation you half remember from 2009. A medication you took for eight months and could not name now. An investigation someone ordered, which you assume came back clear because nobody rang you.

The real answers exist. Your GP record holds the consultations, prescriptions, results and referral letters going back to childhood. But requested raw it arrives as hundreds of pages, part handwritten, part software export, in no order anyone would choose. That is not something you can sit down with the night before an examination and turn into accurate answers.

And when a form is answered wrong, or answered vaguely, the cost is not embarrassment. It is a deferral, a request for more information, and an application that sits still while somebody waits on a letter.

What You Receive

Your complete GP record exactly as your practice released it, alongside a set of documents written from it: a clinical timeline with dates, a plain-English account of your health history, a summary of what is being managed now, and a referenced record summary, with every significant fact citing the page it came from. Handwritten notes are transcribed into readable text.

The page-referencing is the part that matters here. Every date and diagnosis in your pack can be traced to the page of your own record it came from, in seconds, so anything you put on a form you can stand behind.

You sign a letter of authority and we request your full record from your GP practice. Already hold your record? Forward it and skip the wait entirely.

Practices often respond within a week or two, and legally have up to a month. We handle the follow-ups and the chasing.

The work itself takes under a day. Handwriting transcribed, everything checked against the source, every significant fact referenced to the original page.

You Receive Files You Keep Forever

Yours to keep, and to reuse at every renewal, every revalidation and every medical after this one. We securely delete our copy after delivery.

Where the Time Actually Goes

Almost all of the elapsed time is your GP practice releasing the record. That has a legal ceiling of one month, and in practice often takes a week or two. Organising, transcribing and checking your Chronicle Pack then takes under a day of work, whatever size your record turns out to be.

So if your application is already on hold, the question worth asking is when your practice can release your record. If you already hold it, most of the wait disappears entirely.

Who This Is For

Initial Class 1 and Class 2 Medicals

The CAA asks applicants to bring a printout of their GP summary medical history, and reports from their doctor or specialist for any significant past illness. For some conditions the aeromedical examiner cannot complete the assessment at all until those reports exist.

ENG1 and Seafarer Medicals

An unfit or restricted result is recorded and stands for five years unless you can medically evidence that the cause has resolved. It is a decision worth arriving at with your actual history in hand.

HSE Diving Medicals

The medical history questionnaire has to be countersigned by your GP as accurate, precisely because the examining doctor has no prior knowledge of you. Accurate is easier when you have read the record it is drawn from.

DVLA Group 2, D4 and Taxi Medicals

A licence your income depends on, assessed against a history you are asked to declare from memory.

Police, Fire and Ambulance Recruitment

Where an application is deferred pending further medical information, forces are explicit that it is provided at the applicant's own cost and the application waits until it arrives.

Armed Forces Applications

Delays obtaining primary healthcare records are a known bottleneck in recruitment timelines, and the wait is measured in months rather than weeks.

We do not assess your fitness for any role, predict the outcome of any medical, or advise on what to declare, and we have no relationship with the CAA, the MCA, the DVLA, any force, any examiner or Defence Medical Services. Your Chronicle Pack comes to you and nobody else. What you do with it is your decision, and the responsibility for what goes on the form stays yours.

Questions Before Your Medical

Will this help me pass my medical?

We cannot tell you that, and we would not try. We do not assess fitness, predict outcomes or have any say in the decision. What we do is make sure that when you are asked about your history, you are answering from your actual record rather than from memory.

Do you contact my examiner, the CAA, the MCA or my employer?

No. Your Chronicle Pack is delivered to you and to nobody else. We have no relationship with any certifying body, examiner or employer, and we never correspond with one on your behalf. What you share, and with whom, is entirely your choice.

My application is deferred and they have asked for a specialist report. Can you get that?

No, and it is worth being clear about it. We build your pack from your GP record. If you have been asked for a report from a named cardiologist or a specific specialist clinic, that has to come from them. What we can do is give you the complete, dated history behind the request.

How long does it actually take?

Almost all of the wait is your GP practice releasing the record: often a week or two, legally up to a month. The work at our end takes under a day, whatever size your record is, and we will give you a delivery date once we have seen it. If you already hold your record, you skip the first part entirely.

My medical is next week.

Get in touch before assuming it is too late, especially if you already have your record. If it genuinely cannot be done in time, we will say so plainly rather than take the job.

Does it cover hospital and specialist records too?

Your pack is built from your GP record, which includes the letters your specialists and hospitals sent back to your GP, and that is usually the fullest single account of your history that exists. Records held only by a hospital or a private clinic are separate requests to those organisations, and we will tell you if we think something obvious is missing. Making those requests for you is not part of the service today, though we expect it to be later in 2026: there is a short page about it at hospital and specialist records.
